Opening a Treasury Direct Account? A

I found this handy form at the treasury department. I'm just about to fill it out and complete it yet I have two concerns:
I am out of the country and have not yet opened a bank account in the trust's (entity) name. I DO have a bank account in my own personal name, which are nearly identical. Is it acceptable to use my own (the Exchanger's) bank account for this?
