Affidavit of corporate denial

For the sake of having this on questions for people to review, and also would like to know What one legitimately looks like, and if its something I can use in pre-trial/sentencing compliance in 7 days?
I've taken a look at it through a few forums, though they only discuss it more than show what it looks like officially.
