Signature on Bank Account Forms?

Hello,  I am awaiting approval from legal dept to set up account. I just want to clarify when filling out necessary forms if approved, when it comes to signature, should I do my cursive signature with "without prejudice" written underneath it? Or first-middle:last name? The branch manager was questioning why my signature was written that way...
