Dentist office

I just got my trust account opened up. I know that you can use a promissory note to discharge a debt to buy a house or vehicle but I Have a question pertaining to If I want to go to the Dentist office to get work done on my teeth. They would probably look crazy at me if I give them a promissory note. How do i pay them for doing the work on my teeth after they give me the debt amount.....?
