BONDED PROMISSORY NOTE EXAMPLE

If you go to Content/Debt Relief/Discharge Processes and click on the plus sign next to the second folder you'll see a bonded promissory note example. On the second page in the top right corner it says that the DTC account number is the red number on the back of the ss card with a zero placed in front of it.  Is this example accurate and is that the DTC account number?  I've gotten conflicting answers when I've asked that question.
