claim

I recently had an employer not pay me for a months wages. I want to start the process of filing a claim on him. How do I do so. everything I read is inregards to someone filing a claim against you. It is my understanding that I need to file a Notice of Claim, then Notice of fault and finally notice of default. Does anyone know how to do this?
