Form 56 & W8-BEN

I know they don't necessarily go hand in hand but I'd put them in the same ball park. In the "SPC TRUST INSTRUCTIONS" step 2 it says to include the form 56 (e.) and W8BEN and affidavit (f.). Neither of them are included in the collateral of the UCC 1, both are sent to the secretary of treasury along with other documents in step 6. My question is then, where can I get the Form 56 and W8-BEN?
