formula for cusip number

I read that there is a formula for figuring out what the "cusip" number is for our U.S. birth certificates. Is there a formula and what is it? I was told by someone who completed the secured party creditor process that they were instructed to place their social security number and 3 digits from the birth certificate at the bottom of the $100 million Secured Party Creditor bond that gets sent to the U.S. Treasury, which acts as a cusip, is that accurate?
