IRS Voucher (amacd, Tort Division, SFS)

To amacd, Tort Division, SFS, or anyone else knowledgeable, have you had any issues getting the IRS to honor a voucher?

Should one send a notice/affidavit along with the completed voucher with questions (that they won't answer) or should you include statements/facts instead of questions?
