opening a treasury direct account online to link to trust account

I need a little help here. I'm a little more than confused about this process of opening up a treasury direct account online. I did find this form online though it seems pretty simple to follow, but I need to know the exact process to setting up this account so it is linked to my trust account for the interest to hit the account. and to be sure it is done correctly. I already have my trust account set up with my bank i just need setting this up correctly.    here is a link to the form i found on line about opening up a treasury direct account. https://www.treasurydirect.gov/instit/savbond/otc/HowtoopenanaccountinTreasuryDirecttipsheet.pdf   I'm confused by a few of the places to fill in correctly for example: #6 Choose “Individual” as the type of account you would like to open and then click “Submit.” im sure this is where i put trust right #7 Give your account a name is this where we put the name of our trust? I'm not sure Taxpayer Identification Number is this where we put our Trust EIN numbers, or is this where we put our own social security number date of birth  Do i put my own Birth Date or something different here. Everything else is self explanatory to me and i can fill in the rest.  This is what i'm having problems with.  Please help if this is not the proper way to do this then please guide me to the proper channels, or most up to date way of completing this to link it to my trust account.   here is a copy of the form:
